Chiropractic
If you’re animal is currently undergoing veterinary treatment then we will need consent from the vet to carry out chiropractic care. This is not necessary for routine treatment - if your animal is not actively been treated by the vet for a illness/condition. This form can be emailed to mymotionchiropractic@gmail.com or my whats app to 07809 456296. Forms will be uploaded to our clinical software and deleted from phones.
Hydrotherapy
Under the current Veterinary Act all animals must have veterinary consent to swim/treadmill. This is so we can have up to date records of the animal and ensure there are no contra-indications. If you use our equigate we may have sent you a vet report through this. You can also download a report to take into your vets if you prefer.
